PROGRAMME (CDM)
In Ireland today over one million people suffer from a long term condition. These are conditions which are persistent and are long lasting in their effects. They include certain illnesses such as COPD, Asthma, heart disease and diabetes.

The HSE has established an Integrated Care Programme for the Prevention and Management of Chronic Disease. This programme aims to provide better care to people with certain long term illnesses. It allows for care to take place predominantly in a general practice setting and not in hospital outpatients. This will be achieved by providing access to preventative, management and support services to patients via their GP.

Deansgrange Medical Centre is taking part in this programme. If you qualify to participate, our nurses will contact you to invite you to join. There is no charge for the service. For you it will involve visiting the surgery for blood tests once a year and nurse and doctor consultations approximately twice a year. We can also issue your repeat prescriptions during your visits. OPPORTUNISTIC CASE FINDING
PROGRAMME (OCF)
WHAT IS THIS PROGRAMME ABOUT?
This is an opportunity for you to be screened for certain long term illnesses such as diabetes and heart disease. Assessment will also identify if you are someone who may be at a higher risk of developing one of these conditions in the future.

WHY SHOULD I GET INVOLVED?
The aim behind this programme is to identify certain diseases or risk factors for disease early, so that you can be treated and managed in a timely manner for a better health outcome.

HOW DO I GET INVOLVED?
Practically, what it will mean for you is that when you attend your doctor for another issue you may be asked to attend our nurse for an assessment (blood tests and clinical evaluation). Our receptionists will arrange an appointment for you to see our nurse to carry out the assessment. One of our doctors will then review your details and make a clinical evaluation.

HOW WILL IN KNOW MY RESULTS?

If you turn out to have a long term disease you will be contacted and we will facilitate management of that disease in a structured way.

If you do not have a disease but are deemed to be at a higher risk future illness, you will be given help to limit the chances of developing the disease and we will monitor you frequently with annual assessments.

If you do not have a disease and are not deemed to be high risk we will re-assess you again at a further date in the future.
